Client Service Agreement

Introduction

This Client Service Agreement (“Agreement”) is entered into between Be Proactive, a service of YNOT
Imaging LLC (“Company”), and the undersigned client (“Client”).
By signing this Agreement, Client acknowledges that they have read, understood, and agreed to the
terms described below.

1. Scope of Service

Be Proactive provides a home inventory documentation service designed to assist homeowners in
creating a visual and itemized record of personal property within a residence.
The service may include photographing rooms and belongings, documenting visible items, organizing
images into a structured report, and generating estimated replacement values for documentation
purposes.
The final deliverable is a home inventory report for personal recordkeeping and insurance
documentation.

2. Client Authorization to Photograph Property

Client grants Be Proactive permission to photograph and document personal property within the
residence solely for the purpose of generating the inventory report.
Photographs may include personal belongings inside the home and are treated as confidential client
information. They are not published or distributed to third parties.

3. Client Responsibilities

The Client agrees to provide access to the property at the scheduled time, ensure permission to
photograph the premises, notify the Company of areas that should not be photographed, and review
the completed report.
Any errors or omissions should be reported within 14 days of receiving the report.

4. Inventory Estimates

Estimated replacement values are general estimates generated using available data and automated. The report is not a professional appraisal or insurance valuation. Values are provided solely for
documentation purposes.

5. Confidentiality of Inventory Data

Inventory photographs and reports may contain detailed documentation of personal property and are
treated as confidential.
Reports are provided only to the Client. Be Proactive does not share inventory data with insurance
agents, referral partners, or other third parties.

6. Data Storage and Retention

For standard services, inventory photographs and reports are stored for 30 days after delivery and then
permanently deleted.
For subscription clients receiving annual updates, inventory data may be stored on encrypted long-term
storage systems for use in future updates. If a subscription is canceled, stored data will be deleted
according to company policy.

7. Limitation of Liability

Inventory documentation is based on items visible during the service visit.
Be Proactive is not responsible for items not visible, items stored in drawers or safes unless opened by
the client, or changes after the inventory is completed.
Liability for any claim related to the services is limited to the amount paid for the service.

8. No Insurance Representation

Be Proactive does not provide insurance advice or coverage verification. Clients should consult their
insurance provider regarding policy coverage.

9. Cancellation and Rescheduling

Clients may reschedule or cancel appointments with reasonable notice. Additional cancellation terms
may be provided during booking.

10. Record Retention

The signed Client Service Agreement and associated service records may be retained by Be Proactive
for legal, compliance, and recordkeeping purposes.
These records may include the Client’s name, email address, signed agreement, service date, and
related transaction information.

11. Acceptance of Terms

By signing below, the Client confirms that they have read this agreement, reviewed the company’s
Privacy Policy, understood the nature of the service, and agreed to the terms.
Client acknowledges that electronic signatures and digital acceptance of this agreement are legally
binding and carry the same legal effect as handwritten signatures.
